Dr. Lio explains that elimination diets "generally don't work if the only piece is eczema," and he cautions against exclusionary approaches unless there are other symptoms like hives, GI upset, or diarrhea. He warns that cutting foods carries real risk — he has seen children develop protein restriction severe enough to cause kwashiorkor, fall off the growth curve, and fail to thrive — and can even provoke true, dangerous food allergy. Instead, he favors a broad, healthy, anti-inflammatory diet (something "like a paleo diet"), citing the idea that tolerance develops by eating foods, captured in Dr. Ruchi Gupta's line: "Through the skin, allergies begin. Through the diet, they stay quiet."

He frames the barrier hypothesis as a turnaround in thinking — eczema damages the skin barrier, letting environmental allergens sensitize patients through the skin, which is why healing the barrier (as with dupilumab) may help prevent food allergy and the atopic march while children catch up on growth. On supplements, he points to oolong tea after meals, hemp oil, vitamin D, and L-histidine, and describes the skin microbiome as a dynamic "fingerprint" where Staph aureus actively drives disease. For prevention, he notes convincing data for Lactobacillus rhamnosus GG in expectant moms and babies, while established disease calls for blends of strains like L. salivarius and L. paracasei.
